Accessing support and assistance for individuals, families and children

Windermere is a community based organisation delivering a variety of family and community services that can help with specific issues, concerns or needs, including:

  • Support for disability or for children with developmental delay
  • Parenting and family support services including help for children with behavioural concerns and other family related issues
  • Practical and emotional support for victims of crime or those dealing with issues stemming from family violence or other types of sexual assault and trauma. This support can include counselling, coaching, practical and administrative support.
  • Support for people at risk of homelessness.

Referring to NDIS services

Windermere deliver a host of services to those with an eligible NDIS plan. These include, Support Coordination and Specialist Support Coordination and a number of therapy services such as:

  • Speech Pathology
  • Psychology
  • Physiotherapy
  • Occupational Therapy
  • Early Childhood Intervention/Keyworker
  • Exercise Physiology
  • Behaviour Management.

We also offer Plan Management services which can help you if you are struggling with the administration and paperwork that comes with having an NDIS plan.

Understanding the referral process

Referring to Windermere is quick and easy. Simply complete the form below to get your referral underway.

What are the key stages of the referral process?

There are five stages in the referral process:

  1. Complete the form below, including all requested information.
  2. Once you click submit, we are notified and the referral forward to the relevant department.
  3. You should also receive an email confirming your referral details.
  4. We will review the referral information and either contact yourself or the person you are referring directly, depending on the information you have provided.
  5. If we can provide service immediately, we will arrange for the appropriate paperwork to be completed and commence service delivery.

How long does a referral take?

The above process should not take longer than 72 hours (three business days); this may vary depending on complexity of the referral, the situation or the person or family you are referring.
